The page appears to be hosted by ClickBank and contains the standard ordering information form.

They do however strongly recommend to you; once you fill out the form, that you upgrade your free account to their super affiliate package. This upgraded package is said to include a variety of features that are not available to the basic package user. The apparent total cost of their super affiliate package is $77.00 and payment is through ClickBank.
